According to their web site, this sheet is designed for the Gallery Models H-34 kit and is for those that have the V braced landing gear. I do believe that this would also work with the Revell kit that was recently reissued and would be a good choice for the cost-conscious modeler.

There are markings on this sheet for four aircraft, all of them UH-34Ds and all in overall FS 34097 Field Green.

Going by what is on the cover of the instruction sheet, the first up is from HMM 363 'Lucky Red Lions' from 1966. This one has a red rear engine cover and uses the large 'Marines' logo on the fuselage.

Next is one from HMM-362, 'Ugly Angels' and is respendent in white lettering that quickly disappeared after a period of time. This is also from 1966.

Third is the first of two HMM-163 'Evil Eyes' helos. This one carries the small black 'Marines' logo and was featured in a Life magazine article where it was shot down during its March 1965 mission.

Finally, another HMM-163 aircraft, this time one marked with the large eyes on the front of the engine cover. This one is also from 1966. All of these aircraft were flown in Vietnam during these times. Some of them have walkways on the aft fuselage with some requiring the dark grey to be painted on before the white outline is applied.

The decals are superbly printed by Cartograf so you know the quality is first rate. Instructions include a two page data markings guide in addition to full color unique markings placement. An excellent option for your next UH-34D build in this scale.
